* Medical information and physician’s number.
* Parent/guardian signatures for medical and media releases.
* Copy of baptismal certificate for any student preparing for a sacrament:
+First Communion and First Reconciliation - 2nd grade and up
+Confirmation - 9th grade and up
Payment: is by check or cash only, due at time of registration. General registration fees are waived for weeklycatechists, weekly nursery aides, weekly hospitality volunteers, and weekly office aides. scholarships and payment arrangements are available.
Registration fee - $60.00 for one child, $80.00 for two or more
Sacramental preparation fee - $15.00 per child
High school hospitality fee - $10.00 per youth
Generations of faith program fee - $80.00 per family
Generations of faith hospitality fee - $20.00 per family
Non-parishioner fee - $25.00 per family
Late fee (Beginning sept 8) - $20.00 per family
